PHI includes 18 identifiers: name, address, dates, phone/fax numbers, email, SSN, medical record numbers, health plan IDs, account numbers, certificate/license numbers, vehicle identifiers, device identifiers, URLs, IP addresses, biometric identifiers, photos, and any other unique identifying number. Electronic PHI (ePHI) requires additional technical safeguards. PRACTIS encrypts all PHI at rest and in transit using AES-256 encryption and maintains strict access controls with audit logging.
