Under the 8-minute rule, a therapist must provide at least 8 minutes of a timed service to bill one unit. For multiple services, total treatment time is divided: 8-22 min = 1 unit, 23-37 min = 2 units, 38-52 min = 3 units, 53-67 min = 4 units. The rule applies to timed CPT codes (97110, 97112, 97116, 97140, 97530, 97542).
